Wisdom Tooth Pain: Symptoms, Relief and When Removal Is Needed

Why wisdom teeth hurt, how to relieve the pain at home, what impacted wisdom teeth are, what removal involves, how much it costs in India and how recovery goes.

Wisdom teeth, the third molars, usually appear between the ages of 17 and 25. Because modern jaws are often too small for them, they frequently come through at an angle, only partly, or not at all. The result is pain, swelling and infection that bring many young adults to the dentist for the first time in years.

Why wisdom teeth hurt

  • Eruption. The tooth pushing through the gum causes dull pressure and tenderness, much like teething.
  • Impaction. The tooth is blocked by the molar in front or by bone and grows sideways or stays trapped under the gum.
  • Pericoronitis. A partly erupted tooth leaves a flap of gum under which food and bacteria collect, causing a painful, swollen infection, often with a bad taste and difficulty opening the mouth.
  • Decay. Wisdom teeth are hard to clean, so they and the neighbouring molar often decay.
  • Cysts can occasionally form around an impacted tooth and damage bone.

Symptoms to watch for

  • Pain at the back of the jaw, sometimes radiating to the ear or throat
  • Swollen, red gum behind the last molar
  • Bad breath or an unpleasant taste
  • Difficulty opening the mouth or swallowing
  • Swelling of the cheek or jaw, or fever (signs of spreading infection: see a dentist urgently)

Relief at home

  1. Rinse with warm salt water several times a day, especially after eating.
  2. Take over-the-counter painkillers such as paracetamol or ibuprofen as directed.
  3. Apply a cold compress to the cheek to reduce swelling.
  4. Keep the area clean with a soft brush; a syringe of water can flush food from under the gum flap.
  5. Avoid very hot, hard or spicy foods.

These measures relieve symptoms but do not fix an impacted tooth. If pain returns repeatedly, removal is usually the answer.

Does every wisdom tooth need removal?

No. A wisdom tooth that is fully erupted, in a good position, healthy and cleanable can stay. Dentists recommend removal when the tooth is impacted, repeatedly infected, decayed, damaging the tooth in front, interfering with orthodontic treatment, or associated with a cyst. Removal is easier and healing faster in the late teens and twenties, before roots are fully formed.

What removal involves

A simple extraction of an erupted tooth takes a few minutes under local anaesthesia. A surgical extraction of an impacted tooth involves a small gum incision, sometimes removal of a little bone and sectioning of the tooth, and stitches; it takes 20-45 minutes and is done under local anaesthesia, with sedation available for anxious patients. An X-ray or CBCT scan beforehand shows the position of the roots relative to the nerve.

Cost in India

Indicatively, a simple wisdom tooth extraction costs INR 1,500 - 5,000 and a surgical removal of an impacted tooth INR 5,000 - 15,000, depending on complexity, the city and whether an oral surgeon performs it.

Recovery

  • Bite on the gauze for 30-45 minutes and do not rinse or spit for 24 hours, so the clot stays in place.
  • Expect swelling to peak on day two or three and settle within a week; stitches dissolve or are removed after 7-10 days.
  • Eat soft, cool foods for a few days; avoid straws, smoking and alcohol, which can dislodge the clot and cause a painful dry socket.
  • Take prescribed medicines and keep the area clean with gentle salt-water rinses from the second day.
  • Contact the clinic if pain increases after day three, bleeding does not stop, or you develop fever.
